He/She further declares that he/she has received a copy of the relevant general conditions of insurance (GCI) as well as the information sheet "Information about your insurance" and the information provided by the intermediary in accordance with Art. 45 of the Insurance Supervision Act, acknowledges that he remains bound by the application for 14 days unless he has set a shorter deadline for acceptance, and undertakes to pay the premium upon conclusion of the insurance contract.
